ADHD and Autism Questionnaires

Referral questionnaires are part of referrals for ADHD and/or autism assessment.This allows the team to gather the information needed to make the decision to add your child to the wait list for assessment.

Please follow the steps below:

1. Download the documents you require in this section. All documents can be completed on your computer - no need to print them.
2. Complete the questionnaires.
3. Save the completed questionnaires on your computer with the child's name in the file name. Only Word documents and PDF files are accepted.
4. Return completed forms to cwp.aatonlinereferrals@nhs.net 

Help completing referrals

Please note that incomplete referrals are declined after 14 days. This is due to the volume of referrals received. You may wish to complete questionnaires before you start the online referral form. 

School questionnaires are a part of the referral information required. School holidays affect how schools are able to provide this. You may want to start your referral when schools are open to avoid a referral decline for incomplete information.

If you need support with completing the referral and questionnaires, or require the information in any other format, please email the team at cwp.aatonlinereferrals@nhs.net  or you may be able to access support through your education provider.

There are other supporting documents that may be required as part of your child's referral:

Is your child Electively Home Educated (EHE)?

If yes, please ensure to email the team a copy of their latest/initial EHE annual review paperwork.

Has a Knowing Me Profile been completed?

If yes, please ensure to email the team a full copy of their Knowing Me Profile.

Does your child have a First Concerns Plan or a Special Educational Needs (SEN) Plan?

If yes, please ensure to email the team a copy of their latest plan.
