Cheshire & Wirral Partnership (CWP) has worked with Ashfields Primary Care Centre in Sandbach and people with lived experience from our Children, Young People & Families and Learning Disabilities, Neurodevelopmental and Acquired Brain Injury Care Groups to create a new video about the importance of making reasonable adjustments. Reasonable adjustments are small changes that can make a big difference. They help people access services more easily and ensure they receive the support they need in a way that works for them.
By working in partnership and bringing together different ideas, the video shows how reasonable adjustments can improve people’s experiences of care and support. It also highlights the positive impact that listening, understanding, and working together can have on individuals and communities.
This project is a great example of what can be achieved when organisations and local people work in partnership. The video is now available to view on the Cheshire & Wirral website and will be shared with colleagues and partners to support wider learning.
David Crosthwaite, Care Co-ordinator, Ashfields Primary Care Centre supported the project and said:
“The video will be of great support to our community. It introduces clearly and effectively how services in primary care can support our patients to attend their routine appointments, with their personal needs in mind. We hope this video will reassure our patients, that we are here to help support them achieve the right care, at the right place and the right time no matter what their individual needs are."
